Job Description

General Summary:

The role of this position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Iowa and the policies and goals of this facility.

Qualifications:

  • Current RN or LPN License in State of Iowa with no history of abuse.

  • Good assessment skills.

  • Must have proficient computer skills.

  • Good written, verbal and leadership skills.

  • Knowledgeable of OBRA regulations.

  • Must be able to speak and write the English language in an understandable manner.

  • Must be CPR certified.

Essential Job Functions:

  • Complete minimum data set on residents.

  • Complete triggers and raps.

  • Establish standards of care to use as interventions for achieving goals.

  • Complete an individual plan of care for each resident.

  • Communicate care plans to nurse/CNAs; evaluate progress and reset goals.

  • Complete fall, restraint reduction, bowel and bladder assessments and Aims tests.

  • Promote Rehabilitation /Restorative Programs for maximum resident independence.

  • Complete computer entry and transmission of MDS to state repository.

  • Transmit MDS to University of Wisconsin Quality Indicators.

  • Attend conferences/seminars to keep current with regulations.

  • Present/sponsor one in-service per year.

  • Complete Q/A audits and reports monthly; restraint, falls, psychotropic use.

  • Interpret quality indicator reports and set goals for Q/A.

  • Print reports; 672 and 802 for surveys, care plan lists, progress notes and care conference lists.

  • Work Charge Nurse position as necessary.

Physical Requirements:

  • Must be able to move intermittently throughout the day.

  • Must be able to pull, push, move and or lift a minimum of 50lbs to a minimum of 3 feet.

  • Must be able to assist in the evacuation of residents in case of an emergency.
