- Complete minimum data set on residents. 
- Complete triggers and raps. 
- Establish standards of care to use as interventions for achieving goals. 
- Complete an individual plan of care for each resident. 
- Communicate care plans to nurse/CNAs; evaluate progress and reset goals. 
- Complete fall, restraint reduction, bowel and bladder assessments and Aims tests. 
- Promote Rehabilitation /Restorative Programs for maximum resident independence. 
- Complete computer entry and transmission of MDS to state repository. 
- Transmit MDS to University of Wisconsin Quality Indicators. 
- Attend conferences/seminars to keep current with regulations. 
- Present/sponsor one in-service per year. 
- Complete Q/A audits and reports monthly; restraint, falls, psychotropic use. 
- Interpret quality indicator reports and set goals for Q/A. 
- Print reports; 672 and 802 for surveys, care plan lists, progress notes and care conference lists. 
- Work Charge Nurse position as necessary.
